No Surprises Act Notice

The No Surprises Act of 2021 helps to protect patients from receiving surprise medical bills when seeking medical emergency services or certain services from out-of-network providers. This Act requires healthcare facilities and providers to provide patients and the public with notice of the No Surprises Ace and its requirements. 

As such, our office will provide all out-of-network patients with a Good Faith Estimate outlining the anticipated costs associated with any recommendation for care. We will also disclose our fee schedule prior to the commencement of a care plan. The following is a list of insurance plans with whom we are considered out-of-network.

Family First Wellness Center HIPAA Policy

It is the policy of Family First Wellness Center to provide our new patients with our HIPAA compliant forms at the beginning of the first visit. If requested, we keep these forms in a binder behind the front desk. Christy Eakins is the HIPAA compliance officer for maintaining the HIPAA compliance of Family First Wellness Center. Every effort has been made to maintain the privacy of medical records including:

  • Electronic medical information is stored on a server in a closed closet.
  • Records will be kept in digital data format that is secure and encoded.
    A VPN is set up for the transmission of medically sensitive records to
    be transferred safely over the internet.

    Statement of Privacy

    It is the policy of our practice that all physicians and staff preserve the
    integrity and the confidentiality of protected health information (PHI)
    pertaining to our patients. The purpose of this policy is to ensure that
    our practice and its physicians and staff have the necessary medical and
    PHI to provide the highest quality medical care possible while protecting
    the confidentiality of the PHI of our patients to the highest degree
    possible. Patients should not be afraid to provide information to our
    practice and its physicians and staff for purposes of treatment, payment
    and healthcare operations (TPO).

    You as a new practice member have the right to access our HIPAA procedures
    manual at any time.
